In QA before contract is finalized:

  1. Check description of exposure and diagnosis on affidavit and ensure it clearly matches the intake itself

  2. On affidavit if NO PARKINSONS add the code to reference. Tony know what it is.

  3. For address of exposure (ie farm) google the address and make sure it is an actual farm or confirm it is what they claim it to be. Make NOTES in intake. “Exposure address/location verified on Google”

  4. Make sure no sign ups are sent without entire docs completed
