We received criteria and intake question requests from Jon Becker at Meadow Law for the AZ YRTC intake. I highlighted a change made on the criteria per the firm's request. Using the attached files, can you draft an intake that is similar to the attached IL Juv Hall intake example?

@Malissa the campaign is actually AZ YRTC, not JDC. Not sure why Jon listed that as the facilities listed are all in fact YRTC facilities...

We can literally use his exact questions in the AZ intake questions above. Makre sure that Anthony knows that Becker wants "drop downs" as much as possible (limited text fields).

@here Hey guys they gave us 6 tiers in their criteria which includes physical abuse and grooming, just want to make sure that's good to sign and put in the criteria
