@Malissa That version I sent you is my version, i was working on it and made a mix of both their intake form and our old format. This one i'm sending right now is the IL Juv template that we have that they wanted to kind of copy. I'm going to be sending both versions to nick ward as well since I was told he has a meeting with Meadows today
