Over the last 2 weeks, I've been seeing a good amount of DQ's for Roundup exposure. The restriction from DL Flatirons criteria that mandates PNC's need a Parkinson's diagnosis has been a pretty big hang up as well. I've seen a good number of DQ's where PNC's have symptoms and medication but don't have an official diagnosis yet.

@Cameron Rentch VGA AWKO Bradley Grombacher Replacement Requests We received an email from AWKO regarding some signed VGA leads. The email says the signers are now refuting that they ever signed retainers and do not wish to pursue the matter any longer. I have listened to all of the calls. There are no indications that any of these clients were under the influence at the time of signing. It appears as though the signers got cold feet. There is a noticeable 5 month gap between the creation of all of these leads (5 in late July and 4 in mid December). It doesn't appear to be a common pattern. All of the leads are detailed in the spreadsheet linked above. If any additional information is necessary to help make decisions, please let me know!

@Cameron Rentch Law Ruler is showing me 304 signed. According to my communications with Christian Champagne, they've requested a total of 16 replacements all time, however 3 of those requests were files we declined in house and never counted toward the final total. All of that factored in, our number is down to 291 finals. Somehow our numbers are off by 4 finals, and 17 replacement requests. I've checked with Greg Owens to help validate these numbers. His numbers also reflect the discrepancies I just outlined. On the spreadsheet attached, I have highlighted all pending replacement requests in yellow with the email from the firm in the last column. My last communication regarding replacement requests with Christian Champagne was from 1/26/25 regarding Enrico Humphrey's replacement request.

Hi @Cameron Rentch. I compiled a list of all AFFF leads received since 2/1/2025 that have duplicate leads that exist within the system. Some duplicates were created same day, some as far back as 2022. Of the 2282 AFFF leads created in the timeframe, 912 have at least one duplicate lead in Law Ruler. @Cameron Rentch yes! I am working on adding the LeadsPedia ID’s and S1’s now. I’ll have it in order by status. “Send to Five9” is a status we use after a lead has been generated via the API. Leads enter into Law Ruler in the “New Lead” status. That triggers an automation to send the contact information directly into the outbound Five9 dialer and changes the status to “Send to Five9” where it remains until acted upon by an agent.

@Cameron Rentch The lead was initially finalized 03/23/2025 09:05:23 AM. Two hours later at 11:17:50 AM, an intake agent changed the status to Disqualified. The lead was moved today at 10:03:49 AM to Signed e-Sign QA to signify that the lead had been finalized but did not need the automations to be retriggered.

@Cameron Rentch we received a spreadsheet regarding some of the Transvaginal Mesh clients we've signed. I've reconciled the majority of the sheet, however there are 3 discrepancies that I've highlighted. The files say "Closed" on the firm's end, however we have them sitting in Signed e-Sign Final. I believe this communication is their manner of submitting replacement requests. In my review of these files, I found that 1/3 clients (Tiajuana Bethea) had the TVM implanted outside of the 10 year SOL, however this client was signed prior to the criteria update to add that SOL. The criteria update to add the 10 year SOL was communicated 4/18/2025. My assessment of Betty Ann Mitchell's file (based on their notation) is that she has previously received a settlement for this matter, however she did not mention previous representation or legal action taken regarding this matter. The final file, Andrea Cobb, states that her injuries were not related to the mesh, however her alleged complications are within the firm's criteria (pelvic pain & urinary dysfunction).

Please let me know if you require any additional information or if there are any actions I should take regarding these files!

I made sure to emphasize the reasons for each implant, the complications experienced and the revision/removal dates of each implant. The end of each synopsis has a timeline broken down in terms of how the events transpired in conjunction with the SOL. • Original implant within the last 10 years • Revision/replacement or recommendation within 4 years of the assertion of the claim (IE when the claimant is on the phone). Based on this, can you please go back through the replacement/return requests and update your notes and analysis?

Also, we are going to have a couple of additional (minor) changes to criteria coming.

Hey @Cameron Rentch! I was just reviewing the new Transvaginal Mesh criteria. Anapol Weiss shared a sheet with state-by-state SOL's. The Wrongful Death SOL's are clear, however the Product Liability column is confusing. Are those dates referring to the date of the initial implant or the date of the revision/removal surgery? Both of those possibilities heavily conflict with other the parts of the criteria that state the implant must have occurred within the last 10 years and that the revision/removal surgery must have occurred within 4 years of the assertion of the claim.

@Cameron Rentch The total count for NEC finals since 3/1/2025 is 216 across all campaigns. In the intake interview, we do not ask for a distinction between which type of Enfamil/Similac product was used (fortifier vs formula), we only ask for which brand was used. We do ask for that information in the PFS interview. I pulled a report for all completed NEC PFS's and narrowed down for clients that answered Yes to using fortifier and No to using formula. Of the clients that met those parameters, none of them signed between March 1, 2025 and today, however those interviews are ongoing. In the report linked below, the first tab is a list of all Finals between the requested dates. The second tab has a list of clients who have completed the PFS and answered Yes to using fortifier and No to using formula. Please let me know if there's any additional information I can provide! We received an email from AWKO requesting 5 replacements for signed clients. 1 of the 5 was Signed & Declined, never in a billable status so I've omitted that client from this sheet. For the remaining 4, their files all show that the clients meet the necessary criteria with the exception of the DOB based on state SOL. These files are all in the OSOL version of the case type. I have found no other pieces of the criteria that these clients do not meet. Please let me know if you'd like to see any additional information for these clients. I've noticed that many of these files were DQ'd prior to determining if the abuse experienced qualifies because the alleged abuse did not occur at a church related event. Many of them occurred in the PNCs' homes when Missionaries were invited to make their recruitment pitch. Please let me know if you need any additional information about these files. @Cameron Rentch There's definitely some conflicting information in this file. It says the client was incarcerated at Los Padrinos in 2024 but did not experience sexual abuse at that facility. Similarly, they were incarcerated at a Las Vegas juvenile facility in 2022 but did not experience sexual abuse there either. The alleged sexual abuse stems from their 2024 incarceration in San Bernardino Juvenile Hall. The Las Vegas juvenile hall and Los Padrinos were not mentioned during the original intake.

It's possible the Los Padrinos incarceration date is a typo but this should have been caught before the secondary interview was finished processing.

Even the PNC's manual entries from their inquiries detail physical abuse from guards and other staff and not receiving medical attention after the fact.
