AWKO - NEC 2 We're going to do intake and secondary. For the intake, we'll need to add questions to their existing intake (i've attached it). Here's a list of what they want to make sure is covered. If any of these questions are more appropriate on a secondary interview let us know.
The secondary will require getting medical records.
Demographics & Contact Information • Primary parent’s full name, date of birth, and SSN • Other parent’s full name, date of birth, and SSN • If not a parent, their relationship to the injured party and proof of guardianship • Emergency contact name and contact information • Injured party’s full name, date of birth, and SSN • Plaintiff’s phone number, email, and mailing address • Additional phone/email/address for the other parent or emergency contact • If the IP is over 18: their phone, email, and mailing address Birth & Injury/Ingestion Details • Gestational age at birth (and NEC diagnosis) • Birth weight • Birth hospital • If transferred, name of the transfer facility (often this is the NICU) • Approximate date of NEC diagnosis • Approximate dates and brands/types of formula ingested [typically these are found in medical records called “flow sheets”] · Window of pre-diagnosis infant formula (splitting up various brands and sub-brands—i.e., Similac, Similac Special Care/SSC 24, etc.) o Note: fortifiers (often confusingly called human milk fortifiers) are also made by Similac and Mead Johnson but are not infant formulas—fortifiers are typically added to breast milk (including donor milk) or even sometimes formula. These are not cases we’ve accepted unless infant formula was used as the fortifer or there is otherwise proof of pre-NEC dx formula ingestion. · Window of pre-diagnosis human milk ingestion (whether donor milk, Prolacta, or mother’s milk) • Was the mother able to breastfeed? If not, why? • Was donor milk or a donor milk product used? • Was surgery required due to NEC? If yes, what type and how many? · If a gut resection, how much of the intestines were resected (or removed)? · Any related medical procedures? (laparoscopic exploratory surgery, ileostomy, infection-related procedures, etc) • Any other treatment received due to NEC ◦ NEC can lead to massive neurological impairments and infection, so related treatments could include shunt surgery, craniectomy, meningitis treatment, etc. • Current ongoing health issues if any ◦ Can vary, but largely fall in neurological (cerebral palsy) and gastrointestinal (short gut syndrome, colostomy bag, malabsorption issues)

It is coming along, We have sent them the questionnaire back for approval and also secondary questions to review so we have secondary set up properly. Integrations is in communication with their tech dept, and we are setting up a meeting for MR to work with them on medical record retrieval.
is there a guestimate on when the campaign will be done?
once we get approval back from them on the questions it should be fairly quick as Sunnie has the initial intake questionnaire 95% the way done
then we need to build out secondary once that is approved
or knowing what they want to do with the secondary. They might want to skip that step and just have it all on intake. I was pushing for secondary just to increase quality
I just want to make sure we are providing the best quality as this could be a long term good client
sounds like the big thing is we're waiting on them to approve the intake.

it's going lol there's a lot of questions that need to be added, they also need all formatting to be gone for the questions, i'm not leaving until it's finished though

@Malissa from what Andy said, it looked like just the email body wasn’t updated. Criteria is good, intake form is good. We just used an older version for the email body
I reached out to Mark but he didn’t respond so I walked Sunnie through how to update the email body which she did. I was 😵 from moving and my bag was buried in stuff

Everything should be good. I'll be working this weekend now that I can finally relax for a second and not do manual labor in 100+ degree heat lol I'll double check everything else but I'm confident of Sunnie's work
We do need to do a better job of making sure the API docs are updated when we clone campaigns so that Mark has the most up to date questions
Yeah don’t overdo anything right now in Phoenix.

@here Hi! I don't know if the Retainer approval from the attorneys are starting right now but here is what I did for the Dublin and San Bernardino Retainers. Our team will map 2 different retainers, One for Self signer, and one for Rep signer.
• Highlighted in YELLOW are the fields that we we think should be mapped into the self signer retainer • Highlighted in BLUE are the fields that we we think should be mapped into the rep signer retainer Please let me know if this is good so we can proceed mapping these! Do we also want the Intake forms approved?

@deleted-U05496RCQ9M @deleted-U055HQT39PC @Carter Matzinger I just spoke with Bianca at the firm. She wants us to forgo the blue/rep signing retainer for now. She'd like for us to contact them if we get anyone who has a power of atty or is a minor etc. They're expecting most of them to be adults. Also, there's some issues if they're incarcerated.
They'd also like to review our intake questionnaire. You can send them one at a time and i'll send them over for review.
That part of the Dublin criteria about incarceration is the recent change we were told to implement as well
To my understanding they also sent us the same retainer for San Bernardino, is it good on both sides, just having a self signer doc?
